  • Who can be an AFCNS caregiver?

    Caregivers are responsible adults (over age 18) who are not the spouse or legal guardian of the client.

  • Do the client and the caregiver have to live in the same home?

    Yes.

  • Does the caregiver have to own the home?

    No.  The home may be owned or rented.

  • May I apply to take in an adult with a disability to live in my home?

    Yes.  We do sometimes match up clients in need of care with responsible home caregivers.

  • May the caregiver have another job?

    Yes, as long as the job does not interfere with the care of the client.

  • How much does a caregiver get paid?

    The amount varies up to a maximum of $1,700 per month depending on the level of care that the client requires.

  • Who pays?

    The payment is from MassHealth.  The client must have MassHealth standard insurance or a managed care equivalent.

  • What is the screening process for a caregiver and household?

    Home and caregiver screening is done according to the Massachusetts regulations and includes interviews, medical clearance, references, criminal record checks, and home inspections.

  • How should I prepare for an intake meeting?

    Everything that is required to complete the intake process is mandated by MassHealth. This checklist is to help you organize the documents needed when applying for AFC. Your preparation can help make this process as smooth and timely as possible.

    To have ready at the scheduled intake visit:

    1. A copy of the client’s annual physical or wellness visit. Must be within 12 months.
    2. A photo ID of the client and caregiver.

    What to expect at this intake visit:

    • Intake nurse will complete an assessment to determine the client’s level of care.
    • The intake case manager will complete a caregiver and home assessment following MassHealth Regulations.
    • List of all household members.

    Home inspection includes:

    • Working smoke and carbon monoxide detectors.
    • First aid kit.
    • Fire extinguisher (ABC type).
    • All steps both interior and exterior that have 3 or more risers must have at least one railing.
    • The caregiver applicant must have had a physical exam within the last 12 months. AFC staff will provide a form for your Primary Care Provider (PCP) to complete to send to the AFC office.
    • Criminal Offender Record Information (CORI) checks for all household members over the age of 18. This must be done in person and a picture ID is required.
    • Three personal references, either email or mailing address.
