Learn more about our team: Jailyn Youngblood, Care Manager shares her story: I’ve worked in human services for about 20 years, and my journey into Adult Foster Care began in an unexpected way. A former coworker who had joined the team called me one afternoon and encouraged me to apply for an open position. After learning more about their mission and submitting my resume, I knew it was the right fit. Lori Ann Pacheco shares her story: What drew me to Adult Foster Care was their small, personalized approach to care. When someone in the home has a disability, it affects the entire family. I believed that by working closely with families, I could help improve their quality of life and support them in keeping their loved ones at home and thriving in the community. Deanna Benjamin, Director of Operations shares her story: What continues to inspire me in this work is seeing families be able to keep their loved ones at home, and in the community and thriving. Arnaldo Camero-Mallard, RN shares his story: As a nurse, I especially value the opportunity to meet with clients face-to-face, assess their needs in their own home environment, and adapt care to each unique situation. Every day brings new challenges and scenarios, which keeps me learning and growing while making a real impact. Bonnie Griffith, Assistant Program Director shares her story: The work we do changes every day with different scenarios, so I continue to learn and I continue to help people work with different staff. I plan to continue in this field for many years to come.
AFCNS was founded in 2001 by Cynthia Bjorlie, M.D. to provide financial, emotional, and clinical support solutions to families who need care for a disabled or chronically ill loved one.
Our passion and dedication are visible in everything we do. When you work with us, you become part of our extended family and gain a true support system. At AFCNS, we take pride in serving the communities we work and live in. We partner with local agencies to volunteer our time in a variety of ways. Together we can make a difference!
Empowering Independence Through Quality Care
Services We Provide
Health Support
We assign each client a nurse and care manager who visit regularly. Our staff answers health-related questions and provides education, training, and necessary interventions. We offer 24/7 on-call support.
Social Support
We become an extension of our clients’ and caregivers’ families, creating opportunities for community engagement and inclusion.
Family Support
We help maintain and strengthen family relationships and guide clients through the challenges of living with a disability or illness.
Resource Support
Our team connects families with vital resources like Adult Day Health Programs, community-based support, mental health counseling, transportation, and guardianship planning.
Financial Support
We are a MassHealth-funded program. Our caregivers may receive up to $1,700/month per client (plus room and board), based on care level.
