Our team supports adults with disabilities who are living at home with caregivers. For more than 20 years, this MassHealth program has provided health and financial support for our caregivers and clients, to ensure safe living in the community.

AFCNS Provides Support

To help our clients in navigating the complexities of living with a disability.

Clients receive regular visits from trained nurses and care managers. We work with families, medical teams, community resources, and social service agencies toward the goal of keeping our clients with disabilities at home and cared for in a stable environment. Caregivers receive a monthly tax-free payment.
