We look forward to our regular visits with you. This is to clarify what to expect. Our home visits are required by MassHealth–one visit per month for Level 1 and two visits per month for Level 2.
The main purpose of our regular visits is to see how your household is doing. There are other routine things that need to be accomplished. To us better serve you, please:
- Make sure that your client is home. If the client is not there, we will have to reschedule and come back at another time.
- Let your team know as soon as possible if something comes up and the visit must be rescheduled.
- Have the log ready so that we can check it. The log is also mandatory for MassHealth, as it serves as evidence that we are all doing what MassHealth is paying us to do.
- Have your client’s medications available for viewing (the actual bottles and containers). We must check these.
- Make sure your client’s room is accessible. We need to see the room.
- Schedule your next visit before we leave. If it is necessary to change the appointment later, that is fine, but remember that schedule changes affect other families too.
