Who Qualifies

AFCNS' clients are adults over the age of 16 with chronic illnesses or disabilities who are unable to live safely alone and require help with their daily personal care needs.

Our clients live in the private homes of qualified caregivers (who may in fact be a family member) and have the support of our professional staff. It is our goal to keep our clients in their communities so they can continue to grow and thrive.

Clients must be eligible for MassHealth and must need assistance with at least one of the following Activities of Daily Living (ADLs):

  • Walking
  • Dressing
  • Toileting
  • Eating
  • Bathing
  • Transferring

Family Member Solutions

Family Member Solutions - Four Staff memebers in front of shedAt AFCNS, we can help navigate the complexities of living with a disability or chronic illness. Our team-oriented approach focuses not only on our clients’ needs, but the needs of their families and caregivers. By helping to build a strong support network we provide a foundation to maintain and enhance existing relationships.

Each of our clients has a designated care manager and nurse to monitor their needs and progress. We work closely with families and caregivers to ensure they have the resources they need to properly care for their loved ones. We are always available with on-call support — 24 hours a day, seven days a week.

Financial Solutions

Adult Foster Care is a MassHealth-funded program that provides qualified caregivers with a tax-free monthly stipend. Caregivers receive up to $1,500 a month per client plus room and board. Additionally, a family member or other adult in the community may be eligible to be the caregiver of a client already living in his or her home.
